Online Viola Bank Loan Application

The American Viola Society’s Viola Bank reviews applications for determination of financial need.

1. You will need to upload a copy of your photo ID (student ID, copy of driver’s license, or passport front page) to your Member Profile. Look for the file upload tab at the top of the management area. Choose the file to upload, and then click the purple Save & Continue button.
2.  You will need to provide two letters of recommendation from past and /or current teachers, employers, or other responsible people in the applicant’s life. Recommendations should confirm the financial need of the applicant. Recommendations may be submitted on line via this form. or mailed to AVS General Manager Madeleine Crouch, AVS National Office, 14070 Proton Road, Suite 100, Dallas, Texas 75244.
3. Please complete this application in its entirety.
